To submit an entry in more than one category, simply complete the form below, and check the categories you’d like to be considered for. Please note that each category incurs an additional entry fee. To add another entry, click the “Add Additional Entry” button at the bottom of this form. When finished submitting your entries, click the “Proceed to Check Out” button.

Attaching a description is required and helps the judges in the judging process. It can be a simple description of the audience and mission statement or goal of the entry. Please keep your comments under 500 words. Providing a URL to the video is also required.
